1. Assumption of Risk
I understand and acknowledge that boxing, fitness training, and related physical activities involve inherent risks, including but not limited to serious injury, illness, disability, or death. I voluntarily choose to participate in these activities with full knowledge of the risks involved.2. Release of Liability
In consideration for being allowed to use the facilities, equipment, and services at Anchor Boxing, I hereby release, waive, discharge, and hold harmless Anchor Boxing, its owners, employees, trainers, volunteers, affiliates, agents, and representatives from any and all claims, demands, damages, or causes of action arising out of or in connection with my participation in any Anchor Boxing activity.3. Health and Medical
I confirm that I am physically fit and have no medical condition that would prevent me from safely participating in physical activities. I understand that it is my responsibility to consult with a physician before beginning any exercise or training program.4. Personal Property
I understand that Anchor Boxing is not responsible for any lost, stolen, or damaged personal property while I am on the premises.5. Media Release (Optional)
I grant permission for Anchor Boxing to take and use photos or video of me taken in connection with gym activities for promotional or marketing purposes. (If I do not consent to this, I will notify staff in writing.)6. Legal Age / Guardian Consent
I confirm that I am at least 18 years of age. If I am under 18, a parent or legal guardian has agreed to these terms on my behalf.
